Justice of the peace and Form 2

If the police are not able to bring your relative to hospital, the other option is to go to a justice of the peace at the nearest courthouse and request an “Order for Examination” (Form 2). Based on the information provided by you and other family members or witnesses, the justice of the peace decides whether your relative meets the legal definition of being a danger to themselves or others, or of being unable to care for themselves. If so, a Form 2 is completed, allowing the police to bring the person to the hospital for an assessment. It is helpful if you can accompany your relative to the hospital.

This form involves an assessment but does not guarantee an admission to the hospital.
